Student Organization Registration Form

Clubs and organizations must register with the Office of Student Activities at the beginning of the fall and spring semesters in order to maintain recognition and active status with UVa-Wise and the Student Government Association. Completed forms should be submitted to the Assistant Director of Student Activities & Recreation within two (2) weeks of the start of each semester.
