Student Organization Registration Form

Clubs and organizations must register with the Office of the Dean of Students at the beginning of the fall and spring semesters in order to maintain recognition and active status with UVA Wise and the Student Government Association. Completed forms should be submitted within two (2) weeks of the start of each semester.

By completing the registration form, you agree to comply with all policies and practices in the Student Organization Handbook.
