  • Chapter Member Roster: Submit this form at the end of each semester and include active members and new members. This form is used to calculate Chapter GPA.

  • Chapter Officer Form: Submit this form when officers for your organization have changed (either due to elections or vacated positions).

  • New Member Roster: Submit this form at least 2 weeks after the end of Continuous Open Bidding and list all new members who are participating in the organization’s new member program.

  • Permission to Initiate Form: Submit this form at least 5 days prior to scheduled initiation and list all members that your organization plans to initiate.
